Reference Survey

Two references are required. The Reference Surveys must be completed by two individuals who have overseen you in some capacity (supervisor, instructor, mentor, coach, etc.). Each reference will score the applicant on 10 Performance Evaluation Factors. Completed forms must be emailed by your reference directly to Matthew Gault (matthew.gault@ridgewater.edu) by the application deadline. Forms submitted by the applicant will not be accepted.

Prerequisites

Applicants must have the following prerequisite courses either completed or in progress.

BIOL 2100 Human Anatomy (minimum C grade)
CMST 1210 or CMST 2260 (Communication course choice)
PRNU 1617 Medical Terminology
ENGL 1210 or ENGL 1209 & ENGL 0980
MATH 1120 or MATH 1125 & MATH 0995

Students must have a minimum GPA of 2.5 in the prerequisite courses.
